Directions for Arizona Department of Water Resources statement of claimant

Go to AZWATER.gov

1. Click the drop down under “How Do I”
2. On the Left “Apply For” select the bottom option Statement of Claimant
3. Select the Form that applies to what you need
4. Adjudications will come up – select the form that applies
5. Point of Diversion and ditch stats: NW 1/4, SW 1/4, NW 1/4, Section 14, Township 14N, Range 4E
